Veradigm Inc., a publicly traded company that provides practice management and electronic health record services, recently reported a data breach to the U.S. Securities and Exchange Commission (“SEC”) that affected information belonging to patients of Veradigm’s provider customers. Our firm is investigating this incident and would like to speak with people who have received a data breach notice letter or email from Veradigm.
On September 8, 2026, Veradigm filed a Form 8-K with the SEC about a data breach that involved unauthorized acquisition of personal information related to healthcare patients, including Social Security numbers. According to Veradigm’s filing, one of the company’s third-party vendors experienced a cybersecurity incident in which an unauthorized party obtained credentials from the vendor’s environment that could be used (and were used) to access a Veradigm application interface used by the vendor to provide services on behalf of Veradigm’s customers. Using the stolen credentials, the unauthorized third party downloaded copies of patient personal information.
Veradigm said that it is notifying affected individuals, though its investigation remains ongoing. On September 5, 2026, the cybercriminal threat group known as The Gentlemen claimed a data breach of Veradigm’s systems. The Gentlemen claim to have breached 3.5 million patient records, including social security numbers. Veradigm has not confirmed whether the breach claimed by the Gentlemen is the same breach reported in the company’s recent 8-K filing.
