What Is This About?

If you recently received a letter or an email from Rocky Mountain Associated Physicians, P.C. ("Rocky Mountain") informing you that your sensitive personal or protected health information may have been compromised in a data breach, we would like to speak with you about your rights and potential legal remedies. What Happened?

On or after October 30, 2025, Rocky Mountain Associated Physicians became aware of a security incident on its internal systems, including its patient database and other data files. Upon detection, Rocky Mountain launched an investigation with the assistance of third-party cybersecurity experts to determine the nature and scope of the incident.

The investigation determined on February 2, 2026 that an unauthorized third party gained access to certain systems within Rocky Mountain’s databases, including patient records and other files. Rocky Mountain conducted a comprehensive review of the impacted data to determine what information was compromised and to identify affected individuals.

On April 3, 2026, Rocky Mountain Associated Physicians published disclosure of the breach on its website, notified the U.S. Department of Health and Human Services Office for Civil Rights of the breach, and started sending individual notice to impacted individuals.

Upon information and belief, the following types of sensitive personal and protected health information may have been compromised:
