What Is This About?

If you recently received a letter or an email from QualDerm Partners, LLC (“QualDerm”) informing you that your sensitive personal or protected health information was compromised in a data breach, we would like to speak with you about your rights and potential legal remedies. QualDerm recently disclosed that it was the victim of a data breach that exposed individuals’ sensitive personal and protected health information.

What Happened?

On December 24, 2025, QualDerm became aware of unauthorized activity within certain systems in its network. Upon detection, QualDerm promptly took steps to contain the activity and launched an investigation with the assistance of third-party cybersecurity forensic experts to determine the nature and scope of the incident. The investigation determined that an unauthorized actor gained access to a limited number of systems within QualDerm’s network between December 23, 2025, and December 24, 2025, and removed certain information stored within those systems. QualDerm then undertook a comprehensive review and analysis of the impacted data to determine what information was present and to identify the patients to whom the information relates. Although the detailed review remains ongoing, QualDerm has begun notifying individuals whose information has been identified to date. On February 24, 2026, QualDerm sent notification of the incident to the Attorney General of Texas, reporting that over 174,000 residents of Texas were affected by the breach.

What Information Was Impacted?

Upon information and belief, the following types of sensitive personal or protected health information may have been compromised:

            • Doctor Name;
            • Medical Record Number;
            • Date of Death; ;
            • Email Address;
            • Treatment Information;
            • Diagnosis Information;
            • Health Insurance Information; and
            • Government-issued Identification Information (such as a Driver’s License Number) for a limited number of individuals.
