What Happened?

According to a public notice by Ohio Living, an unauthorized actor gained access to specific systems and copied files between April 16, 2026 and April 17, 2026. Ohio Living detected unusual activity on April 17, 2026 and promptly took steps to secure its systems and initiate an investigation into the nature and scope of the issue with the assistance of third-party cybersecurity experts. The organization then began a comprehensive review of the affected files to determine what information was involved and which individuals may have been impacted.

Ohio Living has stated that its investigation remains ongoing and that it is still working to identify the specific individuals affected by the incident and the information potentially affected as to each individual. Ohio Living has, however, determined that a variety of sensitive personal information and protected health information could be among the files accessed or acquired by the unauthorized actor. Ohio Living disclosed the incident on its website on or about June 16, 2026, where it indicated that it will provide direct notice to affected individuals when it is able to do so.
