What Is This About?

According to dark web monitoring sources, Next Level Medical LLC (“Next Level”), a chain of urgent care clinics in Texas, may have experienced a data breach allegedly carried out by the threat group known as PEAR (Pure Extraction and Ransom). Our firm is investigating this incident and would like to speak with current and former patients of any of Next Level’s urgent care clinics.

What Happened?

On or around August 26, 2026, dark web monitoring websites including Ransomware.live detected that the cybercriminal threat group known as PEAR had claimed a breach of Next Level Medical’s systems in a post made to the group’s dark web leak site. PEAR claims to have exfiltrated three terabytes (3 TB) of data from Next Level, allegedly including, among other things, personal and medical information of patients.

As of publication, Next Level has not confirmed the alleged breach’s authenticity. Details of the alleged breach, such the full scope of potentially affected information and the number of potentially affected individuals, remain under investigation.

What Information Was Impacted?

While Next Level Medical has not confirmed the alleged breach nor its associated details, based on allegations made by the ransom group PEAR, the following categories of information may have been compromised:

• Names;
• Medical information/records;
• Health insurance information; and
• Other sensitive personal information and protected health information.
