On or around July 30, 2026, Nephrology Associates posted a public notice of the data breach to its website and sent notification letters to potentially affected individuals for whom Nephrology Associates had address information.

What Information Was Impacted?

According to Nephrology Associates’ public notice, the following categories of personal information and protected health information may have been compromised:

• Name;
• Date of birth;
• Social Security number;
• Driver's license or state ID number;
• Other government ID numbers;
• Treatment or diagnosis information; and
• Health insurance policy information.
