What Is This About?

If you received a letter or email about a data breach from Lake Region Healthcare Corporation (“LRH”), your sensitive personal information or protected health information may have been compromised. What Happened?

On or about May 19, 2025, Lake Region Healthcare detected unauthorized access to its network. Upon discovery, LRH promptly took steps to secure its network and commenced an investigation into the incident with the assistance of external cybersecurity professionals. LRH’s investigation included a comprehensive review of the affected data to assess and identify the individuals and information that may have been impacted, a process which completed on June 5, 2026.

On or around July 1, 2026, LRH filed notice of the breach with the Massachusetts Office of Consumer Affairs and Business Regulation and the Texas Attorney General. LRH has also posted public notice of the data breach to its website. Affected individuals identified in LRH’s review and for whom LRH was able to locate address information have been sent individual notice via U.S. Mail.

What Information Was Impacted?

According to LRH’s public disclosures, the following categories of personal information and protected health information may have been compromised:

                      • Medical record number;
                      • Patient account number;
                      • Health insurance information;
                      • Contact information;
                      • Medical and/or treatment information;
                      • Government-issued identification; and
                      • Financial information.
