What Is This About?

What Happened?

On January 15, 2026, while investigating an unsuccessful phishing attempt, HCCI discovered historical suspicious activity in connection with an employee’s email account. HCCI promptly initiated an investigation with the assistance of external cybersecurity professionals and moved to remediate the unauthorized activity.

The investigation determined that there was a successful historical phishing attack on HCCI that resulted in unauthorized access to the employee’s email account from August 22, 2024 to November 6, 2024. HCCI engaged a third-party data analytics firm to conduct a review of the email account to determine the types of information potentially accessible and to identify to whom the information relates. This review completed on June 11, 2026, at which time HCCI determined that certain protected information may have been subject to unauthorized access but the unauthorized actor.

HCCI began mailing notification letters to potentially affected individuals on July 10, 2026. HCCI has also released a press release about the incident and has posted notice of the incident to its website.

What Information Was Impacted?

According to HCCI’s public disclosures, the following categories of personal and health information may have been compromised:

• Name;
• Date of Birth;
• Social Security number;
• Mailing address;
• Driver's license number/state identification number;
• Payment card information;
• Financial account number;
• Passport number;
• Information about medical treatment, condition, and/or diagnosis;
• Prescription information;
• Health insurance information;
• Provider information; and
• Telephone/fax number.
