What Happened?

Between November 29, 2025 and December 3, 2025, Glendora Surgery Center experienced unauthorized access to a portion of its computer network. Glendora discovered suspicious activity on December 3, 2025 and launched an investigation with the assistance of third-party cybersecurity specialists. The investigation determined that certain patient information, including protected health information, was accessed and taken without authorization during the incident. Glendora disclosed the breach to the U.S. Department of Health and Human Services on March 27, 2026.

On or about May 18, 2026, after completing a review of the affected records to determine the individuals and information involved, Glendora Surgery Center posted a public notice regarding the incident on its website and began sending notification letters to affected individuals.

Upon information and belief, the following types of sensitive personal and protected health information may have been compromised:
