On or about January 16, 2026, FHCSI became aware of unauthorized activity on its network. Upon detection, FHCSI immediately implemented their incident response plan and began an investigation into the nature and scope of the unauthorized access. After the investigation determined on an unspecified date that certain files may have been accessed by the unauthorized party, FHCSI worked to review the potentially impacted files to identify and catalog the types of information present within them and the persons to whom this information belongs. On February 2, 2026, dark web monitoring sites including Ransomware.live reported detection of a claimed data breach of FHCSI by the Termite threat actor group. FHCSI has not commented on whether this alleged breach is the same breach they disclosed in June 2026.

On June 18, 2026, FHCSI notified affected individuals by U.S. First Class Mail, after completing its investigation. On June 22, 2026, FHCSI posted a substitute public notice of the breach to its website.

                        • Date of birth;
                        • Contact information;
                        • Demographic information;
                        • Social Security number;
                        • Medical information; and
