What Happened?

On or about November 27, 2025, through December 3, 2025, an unauthorized actor gained access to certain Excelas systems and may have accessed or acquired a limited amount of information during that time. On or about January 28, 2026, Excelas learned of suspicious activity and launched an investigation with third-party cybersecurity experts and law enforcement.

The investigation determined that the affected systems contained sensitive personal and protected health information and that certain data may have been impacted. Excelas, provides medical record organization and document management services for healthcare providers, insurers, and law firms.

On May 12, 2026, Excelas filed notice of the incident with the Commonwealth of Massachusetts and began sending notification letters to impacted individuals.

Upon information and belief, the following types of sensitive personal and protected health information may have been compromised:

                        • Full name;
                        • Date of birth;
                        • Social Security number;
                        • Government-issued identification information;
                        • Medical, health, and diagnosis information;
                        • Medication information;
                        • Medical record images;
                        • Insurance information;and
                        • Payment information.
