If you received a letter or email from ASC Ortho Management Company, LLC d/b/a Aligned Orthopedic Partners (“Aligned Orthopedic”) about a data breach, your sensitive personal and protected health information may have been exposed. What Happened?

On December 8, 2025, Aligned Orthopedic identified unusual activity within its email environment. An investigation determined that an unauthorized individual gained access to the company’s email environment between November 16, 2025 and December 16, 2025. During that time, certain emails and attached files were potentially accessed.

After completing a review of the affected data on February 17, 2026, the company concluded that sensitive personal and protected health information may have been involved. Aligned Orthopedic mailed notification letters to impacted individuals for whom it has identifiable address information on April 17, 2026.

What Information Was Impacted?

Upon information and belief, the following types of sensitive personal and protected health information may have been compromised:

                      • Driver’s license or state identification numbers;
                      • Medicaid or Medicare numbers;
                      • Financial account numbers;
                      • Medical dates of service;
                      • Medical provider names;
                      • Mental or physical condition information;
                      • Treatment and diagnosis information;
                      • Prescription information;
                      • Health insurance information;
                      • Patient account numbers; and 
                      • Medical record numbers.
