What Is This About?

What Happened?

On or about July 30, 2026, Ultramobile Imaging experienced a network disruption in which protected health information and sensitive personal information was exposed to unauthorized access. Upon discovery, UltraMobile Imaging took action to address the event and investigate its nature and scope with the assistance of third-party computer forensic specialists. The review was ongoing as of September 25, 2026, but UltraMobile Imaging has so far determined that the information potentially impacted includes, but is not limited to, medical information, health insurance information, Social Security numbers, and driver’s licenses.

UltraMobile Imaging submitted notice of the breach to the U.S. Department of Health and Human Services Office for Civil Rights on September 25, 2026, with such notice becoming public on or about October 20, 2026. UltraMobile Imaging has also posted a public notice of the breach to its website. UltraMobile Imaging is expected to notify affected individuals by mail or email upon completion of its review.

What Information Was Impacted?

According to UltraMobile Imaging’s public disclosures, the following categories of information were involved:

• Names;
• Contact information;
• Demographic information;
• Social Security Numbers;
• Driver’s licenses;
• Information related to medical treatment, condition, or diagnosis;
• Health insurance information; and
• Other protected health information and/or sensitive personal information.
