What Is This About?

What Happened?

On April 1, 2026, Oculus Pathology (“Oculus,” formerly known as Clinical Pathology Associates), a physician-owned company that provides diagnostic pathology services to providers in Texas and several other states, discovered suspicious activity related to an employee email account. Upon discovery, Oculus took action to secure its email system and network, and immediately began working with third-party computer specialists to investigate the full nature and scope of the incident. The investigation determined that some employee email accounts were subject to unauthorized access between March 31 and April 2, 2026. While Oculus’ investigation and analysis of potentially compromised data remain ongoing, the investigation has determined that the affected email accounts may contain/have contained personally identifiable information and protected health information.

On or about August 7, 2026, Oculus posted a public notice of the breach to its website. Oculus is providing individual notice to potentially persons.  

What Information Was Impacted?

According to Oculus Pathology’s public notice, the following categories of information may have been compromised:

• Name;
• Date of birth;
• Social Security number/tax identification number;
• Driver’s license or state ID number;
• Financial account number (with or without access information);
• Payment card information (with or without access information);
• Clinical information;
• Medical diagnosis information;
• Medical treatment/procedure information;
• Prescription information;
• Provider name;
• Medical record number;
• Medicare number;
• Patient ID; and
• Health insurance policy number and/or group number.
