What Happened?

Valley Kidney Specialists, P.C. (“Valley Kidney”) became aware of unauthorized activity on its network on or around June 11, 2026. Upon discovery, Valley Kidney took immediate action to secure its network and launched an investigation with the assistance of external cybersecurity professionals to analyze the extent of any compromise of information. This investigation determined that certain files on Valley Kidney’s network were accessed without authorization by a third party on June 11, 2026.

On or around July 28, 2026, Valley Kidney posted a public notice of the data breach to its website (viewable upon visiting the homepage). The notice indicates that Valley Kidney’s review of the affected files remains ongoing. Valley Kidney is expected to issue notice letters to affected individuals once its review is complete.

What Information Was Impacted?

According to Valley Kidney’s public notice, the following categories of personal information and protected health information may have been compromised:

• Patient name;
• Date of birth;
• Address information;
• Social Security number;
• Medical diagnosis, record number, and dates of service/treatment; and
• Health insurance information.
