On October 19, 2025, Unlimited Technology Systems, an Ohio company that provides practice management software for healthcare organizations, discovered unauthorized activity within its commercial datacenter. Upon detection, Unlimited hired external cybersecurity professionals to conduct an investigation, notified law enforcement, and conducted a review of the data involved. The investigation found evidence that an unauthorized actor obtained copies of personal information from Unlimited between October 5 and October 19, 2025. The information copied without authorization falls into the categories of health insurance and patient balance information, medical information, scanned documents, and Social Security number and other personal information.

On July 21, 2026, Unlimited reported the data breach to the Attorney General of Vermont and the South Carolina Department of Consumer Affairs, reporting that 148,342 residents of South Carolina were affected by the incident. Unlimited is believed to have started to send notice letters to affected individuals on or around the same date, July 21, 2026.

According to Unlimited Systems’ sample notice, the following categories of personal information and protected health information may have been compromised:

  • Name;
  • Date of birth;
  • Social Security number;
  • Contact information (email, address, phone number);
  • Demographic information;
  • Government IDs including driver's license;
  • Insurance card;
  • Intake form;
  • Medical diagnosis information;
  • Medical dates of service;
  • Medical record number;
  • Health insurance policy number; and
  • Health insurance claims/benefits information.
