What Is This About?

If you are a current or former patient of Community Health Center of Buffalo Inc. (“CHCB”) of Buffalo, NY, your sensitive personal information or protected health information may have been compromised in a data breach recently disclosed by CHCB. What Happened?

CHCB detected suspicious activity on its network on April 21, 2026. CHCB promptly took measures to secure its environment and launched an investigation with the support of third-party specialists to determine the nature and scope of the unauthorized activity. The investigation determined that an unknown actor accessed CHCB’s network without authorization on April 20 and April 21, 2026. The threat actor accessed certain files that may contain patient information.

CHCB’s public notice on its website indicates that it is still in the process of reviewing affected files and identifying potentially impacted individuals. When the review is finalized, CHCB will notify potentially affected individuals by written letter.

What Information Was Impacted?

According to CHCB’s public notice, the following categories of personal information and protected health information are expected to have been impacted by the breach:

                      • Address;
                      • Date of birth;
                      • Social Security number; and
                      • Driver's license.

                      Medical information, including but not limited to:

                      • Prescription/medication information;
                      • Treatment and treatment location information;
                      • Medical diagnosis/condition;
                      • Lab results;
                      • Medical record number;
                      • Provider name;
                      • Patient medical history;
                      • Health insurance policy information and identification numbers; and
                      • Similar types of information.
