What Is This About?

If you recently received a letter or an email from Advocates for a Healthy Community dba Jordan Valley Community Health Center (“JVCHC”) informing you that your sensitive personal or protected health information was compromised in a data breach, we would like to speak with you about your rights and potential legal remedies. JVCHC recently disclosed that it was the victim of a data breach that exposed individuals’ protected health information.

What Happened?

On December 22, 2025, JVCHC submitted notice of a data breach to the U.S. Department of Health and Human Services Office for Civil Rights Breach Portal. According to the notice, JVCHC has identified 8,842 individuals whose protected health information was involved in the breach. Impacted individuals may soon receive, or may have already received, notice letters or emails from JVHC informing them of their data’s involvement in the breach.

What Information Was Impacted?

Upon information and belief, protected health information was compromised in the breach, though JVCHC has not ruled out the involvement of other sensitive personal information. Protected health information, generally, may include:

  • Contact information such as address and phone number;
  • Medical information such as medical records or prescription information;
  • Health insurance information;
  • Biometric identifiers; and
  • Other personally identifiable information.
